Step 1: Personalized Consultation

Being the most informative and important step, Dr. Katz will have the option to get to know you and your smile goals. We will discuss how our All-on-X full mouth dental implant treatments work and what expectations for the treatment are. We will take computed tomography (CT) scans, X-rays, impressions and digital photographs of your teeth to help build a custom treatment plan that meets your specifications. We will go over the calming sedatives available to you (oral or IV), so you can pick which one you would like. Once you are comfortable with your treatment plan, we will begin planning each step of your surgery to ensure safe and predictable results.

Our All-On-X treatment protocol provides patients with new teeth that are firmly fixed in place, and look and function as close to natural teeth as possible.

A denture, whether full or partial, is a removable set of teeth and should be taken out every night for cleaning. Additionally, since a denture is not fixed in place, it will often slip or sometimes fall out, even with normal use. For this reason, denture adhesives are often needed to hold dentures in place. Even with adhesives, many people find that eating, laughing, being active and sometimes just talking can be a challenge with dentures.

All-on-X provides our patients with a new set of teeth that are connected to the implants and are solidly fixed in place. They may only be removed by the dentist but are brushed and cleaned like natural teeth. Our All-On-X process is the closest you’ll find to natural teeth in terms of look, feel and function.

Since the pressure of biting down is transferred directly to the bone rather than to gum tissue (as it is with dentures), All-On-X also allows the patient to bite with much more force than dentures do. This means that patients can eat their favorite foods without worrying about their teeth!

The short answer is absolutely not. This is the beauty of dental implant treatment vs. dentures. Not only will your implant supported teeth be fixed in place without slipping or needing to be removed for cleaning, they do not press on the gum tissue (which is what creates the discomfort that is common with dentures)!

This line will be hidden behind your lips, so it won’t be visible when you smile. During your initial consultation, Dr. Katz will evaluate your smile to determine where the junction of the fixed bridge and the jawbone will be. During the procedure, we adjust your “smile line,” ensuring that your new smile is not gummy and that the connecting line of your prosthesis is completely hidden.

Published studies indicate a 98.2% success rate when using the All-On-X technique in the lower jaw. Similar studies show a 97% to 98% success rate in the upper jaw for the same procedure. Occasionally, an implant will fail. If this happens, we simply place a new implant in a slightly different position to take advantage of existing bone, then our dental lab refits the fixed bridge to align with the new implant.

Research done by Nobel Biocare and clinical experience shows that in most cases, four implants, specially placed with some angulation, are just as effective in supporting a full arch fixed bridge in either jaw as five, six or eight implant restorations. On rare occasions where bone is soft or worn down, additional implants may be needed, but these situations are the exception, and typically only in the upper jaw. Most dental practitioners do not have the experience that Dr. Katz has with implant surgery treatment protocol and treatment concepts, as they continue to use older techniques involving as many implants as they feel the bone will support. In most cases, more than four implants are not necessary, and can even subject the patient to unwarranted surgeries, excess treatment time, areas that cannot be brushed or cleaned, and higher costs.

Solid foods can be eaten by a patient that has undergone the All-On-X Procedure that same day. However, we are talking about a modified food diet. Defined as foods that are soft enough to be cut with a fork. Examples of the modified food diet are scrambled eggs, meatloaf, mashed potatoes, hamburger, fish and pasta. Please do not eat foods that require any tearing or biting with your front teeth such as pizza, sandwiches and bagels. Also, avoid hard foods such as carrots, chips, apples, nuts and gum. This modified diet is to be maintained until you receive your permanent prosthesis. Once your final bridges are made with the titanium frame as reinforcement, you can resume a normal diet.
On the day of your procedure, we place the implants and a new set of teeth called a “long-term provisional.” This provisional set of teeth is fixed in place, fully functional and capable of providing you with a beautiful smile. You will laugh, talk, play sports and be active without having to worry about your teeth. This provisional set of teeth are made entirely of acrylic resin, which places less stress on your new implants during their first few months of use. We will ask you to be selective in the foods you eat the first few weeks following your procedure, but you will soon be back to a normal diet (without hard or tough-to-chew foods for a while longer). Once your gum tissues have healed and the implants have become completely stable, we fabricate a permanent bridge with acrylic or porcelain teeth. This set of teeth will have an inner titanium framework for extra strength. Think of the titanium as a reinforcement bar that gives your teeth extra support and long-term strength. This permanent set of teeth is carefully attached to your healed gum tissue, and is then adjusted to ensure we achieve an ideal fit in your mouth and that your bite and smile are perfect!
In many cases, where the patient has several broken or missing teeth or an uneven bite, it’s best to do both arches at the same time to avoid compromising the final result. This allows us to control the patient’s bite and biting plane. Doing both arches at the same time is a complex procedure, and is beyond the skill level of most dentists. However, Dr. Katz will use his experience, treatment protocols and expertise to determine if both of your jaw cases (if two are necessary) can be done in one day. In other instances, treatment can be sequenced to meet the financial, health or other individual needs of the patient.
Very few people who need a full arch of replacement teeth are not candidates for the All-On-X treatment protocol. However, each candidate is fully evaluated to determine the most appropriate plan of treatment, in order to address their individual dental needs. One very important note: Dr. Katz requires all candidates to be medically stable before entering treatment. That may entail contacting your physician about any existing medical conditions, and getting your doctor to provide medical clearance. In some cases, our All-On-X treatment protocol is not the indicated approach. Based on dental conditions, some people are better off being treated with conventional dental procedures. We initially determine the appropriate treatment approach at the consultation appointment, then confirm that treatment plan at the prosthetic evaluation.
