All-on-4 and All-on-X replace a full arch of missing or failing teeth with a fixed bridge of new teeth supported by dental implants — teeth that stay in place, that you never take out at night. At State Of The Art Smiles in Pickerington, just outside Columbus, Dr. Phillip Katz plans your case and performs your treatment himself, and your final zirconia teeth are designed and made in our own in-house lab. Dr. Katz has placed more than 10,000 dental implants since 2008, and he has been restoring full mouths since 2000. Consultations are free and include 3D scans, so your plan is built on your actual bone, your bite, and your health — not on a template.

What is the difference between All-on-4 and All-on-X?
Both approaches support a full arch of fixed teeth on dental implants instead of dentures. “All-on-4” describes one specific setup using four implants. “All-on-X” means the number of implants is chosen for your mouth — sometimes four, sometimes five or six — based on your bone, your bite, and how much support your new teeth need. Dr. Katz plans each case individually rather than forcing every patient into the same number, because the right support is part of what makes a full arch last. At your consultation, he will show you on your 3D scans exactly how many implants your case calls for, and why.
Who is a candidate for All-on-4 or All-on-X?
Most adults who are missing most or all of the teeth in an arch, whose remaining teeth are failing, or who are tired of dentures that slip, rub, and limit what they can eat. That includes many people who were told years ago that implants were not possible for them. Bone does thin after teeth are lost, but careful 3D planning can often work with the bone you have, and bone can frequently be rebuilt where it is needed. The only way to know where you stand is a consultation with 3D scans — and ours is free.
How does All-on-X treatment work?
Step 1 — Free consultation and 3D scans. You talk with our team about what is bothering you and what you want your smile to do. We examine your mouth and take 3D scans so we can see the bone and plan accurately.
Step 2 — Your plan, in writing. Dr. Katz explains your options in plain language — including doing less, or doing nothing right now — and you receive the full cost of your plan in writing before you decide anything.
Step 3 — Surgery day. Your implants are placed, with IV or oral sedation if you choose it, and in most cases you leave that day with a fixed set of temporary teeth attached to your new implants. In most cases, you are not sent home without teeth — you leave with a fixed set of temporary teeth the same day.
Step 4 — Healing and your final teeth. Over the following months the implants fuse with the bone and grow strong. Then your final zirconia bridge is designed and made in our in-house lab — in the same building, by the team that knows your case — and fitted.
Why zirconia for your final teeth?
Zirconia is a high-strength ceramic, and your bridge is the part you chew with, speak with, and show the world, every day, for years. Zirconia resists chipping and staining far better than the acrylic teeth used in many full-arch treatments, and it can be shaped and shaded to look like natural teeth. Because our bridges are made in our own lab, the fit and the look are controlled here. If a bridge needs an adjustment, it does not get shipped across the country and back while you wait.
Why patients choose a privately owned office for full-arch treatment
A full arch is one of the biggest decisions in dentistry, and who stands behind it matters. State Of The Art Smiles is a privately owned dental office — not a corporate chain. The owner is the doctor who treats you: Dr. Katz plans your case, places your implants, and sees you at your follow-ups. You work with the same team at every visit, and that team knows your name and your case.
His experience is the other half of the reason. Dr. Katz is a Fellow of the International Congress of Oral Implantologists (ICOI), one of the highest international designations for skill in the surgery and restoration of dental implants. Other dentists learn from him: he was hand-picked by Dr. Frank Spear himself as one of the original 100 Visiting Faculty Mentors at the Spear Institute for Advanced Dental Education in Scottsdale, Arizona, where he also serves as an instructor, lecturer, and mentor, and he served as a Visiting Instructor at the California Implant Institute in San Diego, California. He was named America’s Best Dentist in 2013, 2014, 2015, and 2026, and he has practiced in this community for more than 25 years. This office has served Pickerington since the early 1960s — we have one reputation to protect, in one community, and we protect it by doing right by the people in our chairs.
What does All-on-X cost, and how do patients pay for it?
The honest answer is that the cost depends on your mouth: how many implants your bone and bite call for, whether failing teeth or old implants need to be addressed first, and the sedation you choose. That is why we do not guess. At your free consultation, you receive the full cost of your plan in writing, and if something unexpected turns up during treatment, we stop and talk with you before it costs you anything. Most of our full-arch patients use financing with low monthly payments, which spreads the cost into a monthly amount that fits a household budget. If you have dental insurance, we will verify your benefits and apply whatever it covers toward your plan.
Our patients tell the story best.
All-on-4 and All-on-X questions, answered plainly
More dental implant questions are answered on our dental FAQ page.
How many implants will I need — four, or more?
It depends on your bone, your bite, and the arch being treated. Four implants are enough for some patients; others need five or six for the support their new teeth require. Dr. Katz decides from your 3D scans and will show you the reasoning at your consultation. The number is chosen for your mouth, not set by a package.
Will I be without teeth during treatment?
In most full-arch cases, no. Patients typically leave their surgery appointment with a fixed set of temporary teeth attached to their new implants, so you can smile, speak, and eat soft foods while the implants heal. Those temporaries are later replaced by your final zirconia bridge once healing is complete. If your situation calls for a different approach, Dr. Katz will explain exactly what to expect before treatment starts — you will never be caught by surprise.
I get anxious about dental work. What sedation options do I have?
You do not have to white-knuckle your way through treatment. Dr. Katz is licensed to provide both IV sedation and oral sedation, right here in the office, and he has developed special techniques in sedation dentistry for fearful patients. With sedation, most patients rest comfortably through their appointment and remember little of it afterward. Tell us honestly about your anxiety at your consultation — helping fearful patients is a normal part of what this office does.
What if I was told I don’t have enough bone for implants?
Being told that years ago does not settle it. Bone can often be rebuilt where it is needed, and modern 3D planning can frequently work with the bone you have. Dr. Katz treats complex cases, including patients with significant bone loss. A free consultation with 3D scans is how we find out what is actually possible in your mouth.
How long do All-on-X teeth last?
The implants themselves are built to be a long-term foundation: with normal brushing and flossing, regular checkups, and not smoking, implants can last for decades, and many last a lifetime. The zirconia bridge on top works like natural teeth and can wear over the years, and it can be repaired or replaced while the implants underneath stay in place. Nothing in dentistry is maintenance-free, but a fixed full arch comes closer than any other way of replacing an arch of teeth.
Do you offer payment plans for All-on-X?
Yes. Many of our patients use financing with low monthly payments, which spreads the cost of treatment into manageable monthly amounts. If you have dental insurance, we will verify your benefits and apply whatever coverage you have. At your free consultation, we lay out the full cost of your plan and the payment options together, in writing, so you can decide based on real numbers and your own budget — not on a sales pitch.
